  An Interfaith Pastoral Counseling Center

Pastoral Counseling Services offers a team of highly trained and experienced professional counselors qualified to assess problems, explore solutions and offer a secure framework for personal and spiritual growth. Services include: individual counseling and psychotherapy for adolescents and adults, couples counseling, marital and family therapy, parenting groups, pre-marital education workshops and professional consultation and supervision for clergy and clinical professionals.

Clients’ religious perspectives are respected and may be drawn upon as a therapeutic or learning resource if clients so choose. Our staff do not impose religious beliefs upon others.

Our counselors have graduate degrees. They are professionally certified and/or licensed by the State of Florida. They are members of the American Association of Pastoral Counselors (AAPC), the American Association for Marital and Family Therapy (AAMFT), the National Association of Social Workers (NASW), and the American Mental Health Counselors
Association (AMHCA).

A History of Quality Care

Pastoral Counseling Services, formerly Samaritan Counseling Services, was established in 1971. For over thirty years we have been providing reputable care to the community of Northeast Florda. With all counselors licensed and/or certified, services continue to meet high professional and ethical standards.

Financial Arrangements

A standard fee of $110 per counseling hour is charged for services. Adjustments may be made on the basis of personal financial resources and as the Center’s charitable funding allows. Health insurance is applicable in many cases.

Location

We are located in the San Marco area of Jacksonville, convenient to downtown and major thoroughfares. Our address is 2140 Mango Place. We are two short blocks east of Hendricks Avenue and a block and one-half south of Atlantic Boulevard.

Staff

Louis R. Lothman, Th.D., L.M.F.T
Executive Director

Certified pastoral counselor and Florida licensed marriage and family therapist (#MT1294). Doctor of Theology in Pastoral Counseling from Columbia Theological Seminary. Post doctoral training at the Institute for Contextual Growth in Philadelphia, the Washington School of Psychiatry and the Institute for Relationship Therapy in New York. Dr. Lothman is a Presbyterian minister, a Fellow of the American Association of Pastoral Counselors, and a clinical membr of the American Association for Marriage and Family Therapy.

Christine Oakley, M.A., L.M.H.C.
Operations Director

Florida licensed mental health counselor (#MH6864). Master of Arts in Counseling Psychology from the University of North Florida. Ms. Oakley is a Member of the American Mental Health Counselors Association.

Susan M. Martinez, M.S.W., L.C.S.W.
Staff Therapist

Florida licensed clinical social worker (#SW5659). Masters of Social Work from Florida State University. Ms. Martinez is a Member of the National Association of Social Workers and Northeast Florida Association for Marriage and Family Therapy.

Ross E. McDonough, M.S.W., I.S.W.
Staff Therapist

Florida registered social worker intern (#ISW 2246). Master of Social Work from New York University. Graduate internship at the Mount Sinai Medical Center, Department of Ambulatory Psychiatry. Mr. McDonough is a Member of Northeast Florida Association for Marriage and Family Therapy

Rachel Weinstein-Abrams, M.A., L.M.H.C.
Staff Therapist

Florida licensed mental health counselor (#MH7207). Master of Arts in Mental Health Counseling from Rollins College. National Certified Counselor and Board Certified Music Therapist. Ms. Weinstein-Abrams is a Member of the Northeast Florida Association for Marriage and Family Therapy and the Florida Association for Music Therapy.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

As a client at Pastoral Counseling Services, we keep medical information about you. This information is personal and private. We need to use this information in many ways. First, we use the information when we treat you or refer you for treatment.

Second, we use the information in seeking payment for your care. Finally, we use this information for our health care operations and quality assurance.
Under the law, each patient has certain rights to the medical information kept at Pastoral Counseling Services. The rights are:

bulletAccess: You can ask to look at your information.
bullet Restriction: You can ask to limit who is provided your information. You can ask to limit what information is released.
bullet Accounting: You can ask to see the list of places where your information has been sent.
bulletAmending: You can ask to change medical information if it is wrong.

Florida Statutes: Florida statutorily grants patients the right of access to medical records maintained by health care practitioners. The disclosure of patient information by providers is generally prohibited without the patient's consent, subject to specified exceptions. Florida also has numerous laws protecting the confidentiality of health information held by a variety of entities and government agencies.
